Gleevec Has Activity for Treatment of Aggressive Fibromatosis

A multicenter trial has determined that Gleevec (imatinib) treatment of advanced aggressive fibromatosis (desmoid tumor) is associated with a 16% response rate and a disease stabilization rate of 68%. The details of this study appeared in the March 1, 2006, issue of the Journal of Clinical Oncology .

Fibromatosis or desmoid tumors are one the many soft tissue sarcomas that occur in both adults and children. The usual sites of development are in the abdomen and in the extremities. Primary treatment is with surgery and/or radiation therapy. Local recurrences are said to be relatively frequent and these tumors are considered insensitive to standard chemotherapy agents.

Gleevec is a relatively new biologic agent that was initially approved for the treatment of chronic myelogenous leukemia and gastrointestinal stromal tumor (GIST), another type of sarcoma. Gleevec is currently being evaluated in clinical trials for the treatment of various cancers, including sarcomas.

Gleevec is a tyrosine kinase inhibitor and one function of Gleevec involves binding to c-KIT that contains mutations involved in GISTs. This binding action stops or slows the excessive growth of the cancer cells caused by the mutated c-KIT and has also been implicated in the direct death of cells containing mutated c-KIT.

Researchers involved in this study treated 19 patients with high doses of Gleevec (800 mg/day). Thirteen of these patients had failed prior chemotherapy regimens usually containing anthracyclines and ifosfamide. They reported a partial response in 3 patients and disease stabilization in an additional 13 patients. The one year disease control rate was 37%. Pharmacological studies suggested the kinase pathway that Gleevec inhibits. This may allow future studies of ways to predict which patients will respond and which will not. Gleevec was well tolerated.

Comments: This study adds to the growing list of tumors that respond to Gleevec and may help point the way to other targeted therapy.
